Ivermectina y covid: lo que sabemos del medicamento y lo que dicen los médicos
"No previene el coronavirus, hay un uso abusivo y no racional". El médico infectólogo Rodolfo Buffa advirtió sobre los riesgos de recomendar y consumir esta medicina contra parásitos como protección ante el nuevo virus. Puede causar serios efectos colaterales, como hepatitis.

La ivermectina, un fármaco con amplio uso en animales y en algunas patologías provocadas por ectoparásitos en humanos como piojos o sarna, es recomendada como protección y terapia contra la covid -19.
Entre todos los medicamentos que surgieron en la pandemia para combatir el avance nuevo virus, la ivermectina se hizo famosa; en las redes sociales, en WhatsApp y en la calle se habla de ella y se recomienda para evitar consecuencias graves del coronavirus.
En San Francisco se disparó la venta de este antiparasitario y llegó a tener una demanda del 100% en las farmacias, ya sea en su formato de gotas o en comprimidos.
Sin embargo, su eficacia para prevenir y tratar la covid no está científicamente comprobada. Pese a que existen usos aprobados de la ivermectina en personas y animales, y diferentes estudios científicos la muestran como un potencial inhibidor del SARS-CoV-2, aún no está aprobada para la prevención o el tratamiento del coronavirus.
Así lo aseguró en diálogo con LA VOZ DE SAN JUSTO el médico infectólogo Rodolfo Buffa, quien remarcó que esta droga "no tiene ningún efecto en el tratamiento ni en la profilaxis contra el virus, no hay absolutamente nada demostrado. No se debe tomar ivermectina, ni para tratar ni para prevenir el coronavirus".
El médico advirtió que se está realizando un "uso abusivo y no racional de la ivermectina" que puede causar serios efectos colaterales entre ellos, hepatitis, y explicó qué otras patologías pueden tratarse con este medicamento en los humanos.
- ¿Qué es la ivermectina?
Es un antibiótico
antiparasitario que existe desde hace mucho tiempo. Tiene amplio uso en
medicina humana y veterinaria, con dosis definidas para conservar un patrón de
tolerancia y seguridad para indicaciones conocidas. Su descubrimiento fue
anecdótico, ya que unos japoneses que jugaban al golf encontraron un hongo con
el que comenzaron a producir este medicamento, que se utiliza para el
tratamiento de parásitos.
- ¿ Para qué patologías se indica en humanos?
Tiene uso probado para tratar parásitos, fundamentalmente relacionados con el ámbito veterinario; específicamente tratamiento de parásitos que afectan al ganado vacuno. Pero con el tiempo también se la utilizó para tratar de algunos ectoparásitos en los humanos, las pulgas, los piojos, las ladillas y algunos tipos de sarnas, esos son los usos probados en la actualidad. Tiene propiedades antivirales pero in vitro, no en el ser humano. Es importante detallar lo que este medicamento tiene probado, hay confusión en la gente y se hace un uso abusivo y no racional de la ivermectina actualmente, especialmente en San Francisco. Demostró ser un buen antiparasitario, bueno y seguro para las patologías probadas como piojos, sarna y otros ectoparásitos, en las dosis que están formuladas y estudiadas.
- ¿Qué papel puede tener en el tratamiento y la prevención del coronavirus?
No tiene ningún efecto en el tratamiento ni en la profilaxis contra la covid, no hay absolutamente nada demostrado. No se debe tomar ivermectina ni para tratar ni para prevenir el coronavirus, si se hace, es bajo la responsabilidad de quien lo indica y quien lo toma. No se ha probado para nada que la ivermectina sea un antiviral válido en el ser humano. Sí se ha demostrado que es un antiviral in vitro, en el laboratorio, en lo que se llama la fase 1, pero no existe en el resto. Según la Sociedad Argentina de Infectología (Sadi), la ivermectina es un potencial inhibidor de la replicación viral del SARS-CoV-2. Sin embargo, la evidencia disponible in vitro sugiere que para alcanzar niveles efectivos de ivermectina se necesitarían importantes aumentos y potencialmente tóxicos de la dosis. Incluso, dosis de hasta 10 veces mayores que las aprobadas no alcanzarían las concentraciones efectivas in vitro contra el SARS-CoV-2.

- ¿Cuáles son los efectos secundarios de esta droga?
No es seguro este medicamento para la supuesta función de tratamiento y prevención de covid. Todo lo contrario, tiene descriptas altas probabilidades de producir intoxicaciones, entre ellas hepatitis grave. Algunos de los efectos secundarios que pueden asociarse con la ivermectina incluyen sarpullido, náuseas, vómitos, diarrea, dolor de estómago, hinchazón del rostro o de las extremidades, eventos adversos neurológicos -mareos, convulsiones, confusión-, baja repentina de la presión arterial, sarpullido grave que pueda requerir hospitalización y hepatitis. Los veterinarios advirtieron durante mucho tiempo con preocupación sobre el uso indiscriminado de la ivermectina ya que lleva a la producción de resistencia para los tratamientos para las patologías para las que se indica. La ivermectina es segura para lo que está formulada, no para lo que no se sabe y eso incluye la Covid-19.
Se fabrica en San Francisco
En la Planta Municipal Elaboradora de Medicamentos que funciona en el Parque Industrial de San Francisco se produce ivermectina, aunque desde antes de la pandemia.
En las partidas presupuestarias correspondientes al área de Salud de 2021, se incluye a la ivermectina en la producción junto al alcohol en gel, vitamina D, repelente para mosquitos y otros medicamentos que ya se vienen fabricando y distribuyendo desde la farmacia del municipio.

¿Cómo empezó el uso contra el virus?
Todo empezó el 3 de abril, cuando en una pre publicación, la científica australiana Kylie Wagstaff comunicó que usando una cantidad de ivermectina muy alta (aproximadamente 50 veces la dosis que se usa comúnmente), la medicina era capaz de impedir que el SARS CoV2 ingrese a un cultivo celular.
Eso es lo que se llama un estudio in-vitro, es decir en un tubo de ensayo. Dos semanas después, médicos de Bangladesh comunicaron que habían logrado "curar" al 98% de pacientes que recibieron la combinación de ivermectina y el antibiótico doxiciclina.
En tanto, la administración de esos medicamentos no fue parte de un estudio. Más del 90% de pacientes con covid se recuperan espontáneamente. Es posible que los pacientes pudieran haberse recuperado sin el uso de esas medicinas.
Posiblemente motivados por el estudio de la doctora Wagstaff y el reporte de Bangladesh, y en medio de la urgencia de encontrar medicinas efectivas contra el nuevo coronavirus, es que médicos en Perú, Bolivia, República Dominicana y México empezaron a promover el uso de la ivermectina para la prevención y el tratamiento del virus. En los meses siguientes, el uso no aprobado se extendió a casi todos los países de América Latina.

Historia de un medicamento revolucionario
La historia de cómo se descubrió la ivermectina es bastante increíble. A finales de los 60, Satoshi Ōmura, un microbiólogo del Instituto Kitasako de Tokio, empezó a coleccionar miles de muestras de suelo de todo Japón, a la búsqueda de compuestos antibacterianos. Cultivaba bacterias provenientes de las muestras, identificaba los cultivos con potencial médico, y las enviaba a 10.000 km de distancia a los laboratorios Merck en New Jersey, donde su colaborador William Campbell probaba su efecto contra lombrices parasitarias que afectan al ganado y otros animales.
Un cultivo proveniente de una muestra recogida cerca de un campo de golf al sur de Tokio mostró un efecto notable contra las lombrices. La bacteria en el cultivo resultó ser una nueva especie, que bautizaron como Streptomyces avermictilis. El compuesto activo, llamado avermectina, fue modificado químicamente para aumentar su actividad y seguridad.
El nuevo compuesto, llamado ivermectina, se comercializó para uso animal en 1981 y pronto se convirtió en uno de los fármacos veterinarios más vendidos en el mundo. Es extraordinario que, a pesar de décadas de búsqueda, S. avermictilis siga siendo la única fuente de avermectina que se ha encontrado.

Campbell instó a sus colegas a estudiar la ivermectina como posible tratamiento contra la oncocercosis (también conocida como ceguera del río), una enfermedad devastadora causada por lombrices y transmitida por moscas, que dejaba ciegas a millones de personas, sobre todo en África subsahariana. Los primeros ensayos clínicos en Senegal mostraron la eficacia del tratamiento, y la ivermectina se aprobó para uso humano en 1987. Desde entonces, se han distribuido más de 3.700 millones de dosis (donadas por Merck) a través del mundo en campañas de administración masiva contra la oncocercosis y la filariasis linfática (otra enfermedad causada por lombrices que provoca una hinchazón importante de las extremidades).
El impacto de la ivermectina en disminuir la carga de estas enfermedades devastadoras es inmensurable. Merecidamente, el premio Nobel 2015 para fisiología y medicina se otorgó a Ōmura y Campbell "por sus descubrimientos de una nueva terapia contra infecciones causadas por gusanos nemátodos" (lo compartieron con Youyou Tu, quien descubrió el antimalárico artemisinina).
La ivermectina es un fármaco revolucionario con muchos usos potenciales. Fue el primer "endectocida" del mundo -un fármaco activo contra una gran variedad de parásitos internos y externos, desde nemátodos hasta artrópodos, también ha resultado ser sorprendentemente seguro para humanos.
