El invierno de Juan Ignacio con dermatitis atópica
Tiene 5 años y sufre esta patología dermatológica no contagiosa que le impide hacer las mismas cosas que otros niños, sobre todo en invierno, que agudiza los síntomas de irritación y puede desencadenar en la aparición de eccemas que pican y lastiman la piel.

Los brotes en la piel pican y duelen. La picazón es tan intensa que por momentos se vuelve insoportable y el rascado incontrolable provoca lastimaduras e infecciones. La dermatitis atópica tiene gran impacto en la calidad de vida de quienes la sufren.
Cuando hace frío, los síntomas empeoran, porque la piel sensible reacciona ante los cambios bruscos de temperatura.
El sanfrancisqueño Juan Ignacio Cremasco, de 5 años, tiene dermatitis atópica desde que nació, aunque le fue diagnosticada recién al año y medio. Su mamá Cecilia Villarreal contó en diálogo con LA VOZ DE SAN JUSTO cómo vive con esta enfermedad inflamatoria crónica, no contagiosa, que altera el sistema inmunitario y, en su variante severa -como la de él-, se manifiesta frecuentemente con brotes en la piel, picazón intensa y persistente, dolor, enrojecimiento, costras, secreciones e infecciones.
Juan sabe que tiene esta enfermedad y, como cualquier chico de su edad, no para de jugar pero no puede hacer las mismas cosas que otros niños. Cecilia aseguró que la forma severa afecta su calidad de vida, que siempre tratan de mejorar. Remarcó que "no es contagiosa" y brindó su testimonio para concientizar y visibilizar la dermatitis.
"Es una enfermedad que impacta no solo en la vida de quien la sufre sino en toda la familia. Juan Ignacio está muy restringido en muchas cosas, la dermatitis le apareció en muchos lugares del cuerpo. Es un acompañamiento constante y hay que adaptarse, los padres tenemos que informarnos y ocuparnos de que tenga la mejor calidad de vida posible", aconsejó.
Cecilia afirmó que al rascarse mucho, el pequeño muchas veces está lastimado en los antebrazos y detrás de las rodillas. Esa situación a veces despierta temor en las personas que lo ven y no conocen sobre esta enfermedad. Por eso quiere que todos sepan qué es la dermatitis. "Nos pasa que otros padres cuando lo ven lastimado, lo miran raro, pero hay que explicarles que esta enfermedad no se contagia".
Según la Asociación Civil de Dermatitis Atópica Argentina (Adar), integrada por pacientes y familiares afectados por la enfermedad, 4 de cada 10 personas con la forma severa sienten vergüenza y 1 de cada 2 se frustra por la patología con frecuencia o siempre, a la vez que produce ausentismo laboral y escolar: los adolescentes en promedio pierden 26 días de clase al año (3,5 días por cada episodio).
Brota con el frío
Especialmente cuando hace frío, los brotes y la picazón llegan con más frecuencia. Para controlarlos, Juan Ignacio debe usar una "batería" de cremas que tiene que ponerse todos los días, además de tomar medicamentos porque también sufre de alergia a algunos alimentos, los ácaros, el polvillo y la humedad.
"La peor época es el invierno por el frío, cuando llega el calor tiende a mejorar, quizá hace dos brotes en el verano pero en el invierno los tiene más seguido. Siempre está lastimado en los antebrazos y detrás de las rodillas, donde aparece la dermatitis más comúnmente".
Cecilia afirmó que las cremas hidratantes y emolientes, debe aplicárselas todos los días. "También se coloca vaselina sólida para prolongar el efecto húmedo en la piel, tiene que usar un jabón de baño que no tenga alcohol y un shampoo especial. En el verano debe usar protector solar y repelente para los mosquitos porque las picaduras le provocan infecciones. Hasta una lastimadura puede significar en él una infección más peligrosa", dijo.
La picazón le afecta el sueño y todavía duerme con sus papás que controlan el rascado incontrolable en las noches. No puede estar en un ambiente muy calefaccionado o sobreabrigado, el baño tiene que ser día por medio y con agua tibia, no se puede frotar. Cuando va a los cumpleaños no puede pintarse la cara, como la mayoría de los chicos, sino que el dibujo lo lleva en el dorso de la mano donde su piel no tiene tanto problema.
Su mamá Cecilia dijo que en su habitación "todo es hipoalergénico, la ropa que usa es toda de algodón, por lo cual es muy difícil abrigarlo. En el jardín no puede jugar con arena o tierra y muchas veces tiene que faltar por las lastimaduras porque el peligro son las infecciones en la piel".
Desde el diagnóstico hasta la actualidad el pequeño realiza el tratamiento en el Hospital de Niños de Córdoba y actualmente en la ciudad con la médica dermatóloga Verónica Montiel del Sanatorio y Clínica San Justo.

Los primeros síntomas
El pequeño Juan Ignacio presentó síntomas de la dermatitis apenas nació, aunque recién fue diagnosticado cuando tenía un año y medio. Su mamá contó que tenía eccema en la cabecita y en los dedos y que lloraba todo el día, la única forma de calmarlo era cuando le daba de mamar.
"Su piel estaba muy seca, lo tocábamos y parecía estar tocando cartón. No paraba de llorar y al año comenzó a rascarse sin parar, se puso todo colorado como si se hubiera quemado, me dijeron que era una reacción alérgica a lo que había comido en un cumpleaños. Hizo un tratamiento con dieta y corticoides pero no mejoró, lo aislaron y no pasaba nada, seguía la picazón", recordó Cecilia.
Agregó que como no había mejoría consultaron a un alergista. "Le hicieron un estudio del nivel de alergia que le dio valores muy altos, además le examinaron la piel y me dijeron que tenía dermatitis atópica. Era la primera vez en mi vida que escuchaba hablar de esta enfermedad, no tenía idea de lo que era".
"Verlo todo brotado y lastimado es terrible", dijo Cecilia y recordó que el pequeño muchas veces que no podía caminar porque estaba en "carne viva".
Recordó que mientras tenía ropa puesta no llegaba a lastimarse de la misma manera que lo hacía cuando se rascaba cuando le sacaba la ropa para cambiarlo. Afirmó que para evitar el rascado le fabricó un "pantalón con patitas".
"Le cosí las medias al pantalón para que el no se lo levantara y se rascara. Después terminó siendo un mameluco, porque tuve que coser la remera al pantalón y ponerle botones atrás. Era la única forma de que no se levantara la ropa y se desfigurara", relató.

mayormente en la infancia
La dermatitis atópica aparece mayoritariamente en la infancia y, en un porcentaje importante de casos, con los años el cuadro se va revirtiendo, pero en 3 de cada 10 pacientes la condición continúa tras la pubertad y algunas personas inclusive experimentan los primeros síntomas recién en la adultez.
Se presenta como una picazón intensa y enrojecimiento de ciertas zonas de la piel como el cuero cabelludo, la cara, el cuello, la zona de pliegues o como eczema crónico de las manos y también puede comprometer la zona genital, pezones y areolas y provocar un gran impacto en la calidad de vida.
Su prevalencia está en aumento y son relevantes en la actualidad, el gran impacto en la calidad de vida de los pequeños pacientes y sus familias.
La médica especialista en Dermatología, Verónica Montiel (MP-25408/7; ME-10550) afirmó que el tratamiento de esta enfermedad, independientemente de la gravedad, "debe ser implementado para cada paciente diagnosticado con dermatitis atópica".
Añadió que las estrategias de manejo de la DA, "deben incluir el cuidado apropiado de la piel -la hidratación y humectación aplicadas a toda la piel-, medidas antisépticas y la evitación del disparador, evitar irritantes en general, como se identifica para cada paciente, con tratamiento agudo añadido según sea necesario para brotes, es decir, intensificación aguda de los síntomas y la inflamación".
Explicó que el tratamiento de los brotes agudos: dependiendo del paciente y de la preferencia del médico, "puede ser controlado generalmente, con un corticosteroide tópico. La terapia subsiguiente de mantenimiento depende entonces de la gravedad, así como de la persistencia de los signos y síntomas de la enfermedad".

Cuidados
en esta época
Cuando comienza el invierno, el frío aumenta los brotes y agudiza los síntomas de irritación por la Dermatitis Atópica mientras que los ambientes cerrados y climatizados resecan la piel, empeorando el cuadro de quien posee eccemas.
A raíz de ello, es importante insistir en los cuidados que la piel sensible necesita en esta época del año:
•Mantener los ambientes húmedos (colocando en cada uno un recipiente con agua).
•Realizar baños cortos, con agua tibia.
•Utilizar jabones sin perfume.
•Aplicarse abundante crema humectante, tras secarse la piel con una toalla suave.
•No sobreabrigarse.
•Vestirse en capas, con ropa fácilmente removible.
•Evitar el aumento de la temperatura corporal y la sudoración.
